I published a form through jotform on June 26, 2017, but the form can no longer be found. I have not disabled the form and never selected a deadline date for the form. How can enable the form again?

Your form is not lost, however, your account has been suspended due to suspected phishing activity.
These fields will need to be removed in order to re-activate your account. You can use one of our many payment integrations to collect payments instead.
