Is there a way to access the form to make sure the form is functioning correctly? Our web guy never gave us any type of login to the form and it isn't working. He is no longer working for us. How can we correct this issue

The form is under the account username "nerdout", if you are not the owner of the account, there is nothing we can do, you will need to contact the owner of the form, and ask him to contact us to request the transfer of that form to your own account username.
