How do I get conditions to work correctly?

In the re-enrollment form at the URL below, when the answer to "Please indicate your plans..." is "I plan to return...", only the submit button should appear after this question. The form worked correctly in May, but now it does not. I tried to fix it but now it is worse.

When the answer is "I will not return..." then immigration info, mailing info, submit button should appear.

If "I plan to transfer.." is selected under immigration info, additional conditions should appear (this seems to be fine) and the submit button should appear.

I have been checking your conditions and it seems like you have updated your field "Please indicate your plans for the September/October session below. " since some conditions to not seem to have an option in the statement.

I would recommend you to select a valid/existing option or delete the condition, otherwise it may cause conflicts with the other conditions on your form. Please, also note that you need to update your conditions if you update the related field in the statements.

Also, when you open the wizard (conditions) it shows correctly, you need to click on the edit button in order to see the missing fields.

Please, let us know how it goes.

Since this form is due from our students by Monday, July 24, and in order to avoid losing information or adding more confusion for our students and staff, I cloned the re-enrollment form to use the next time it is to be sent. However, the cloned form will not allow me to see or edit conditions.
The only updates we made to the field below was to change the session to September/October in the question and answers, and change the date in “I plan to return…” to July 28. This should not impact the conditions we had previously set and that worked correctly when we sent it in May.
We need to get this fixed and to prevent the continued malfunction of the conditions options, which periodically occurs on this form and others we rely on.

I have cloned your clone version, and it allows me to see and edit conditions, example:

On regards of displaying only the submit button if the answer to the "Please indicate your plans for the September/October session below.", is equal to "I plan to return", you nee to do the following changes:

    RE: the original Re-enrollment form.
When you get to “Please indicate your plans…”, nothing should appear until you answer the question.
If the answer is “I plan to return..”, then only the submit button should appear. Right now, Immigration Info also appears and it should not.
If the answer is “I will not return…”, then only Non-returning student info, immigration info and submit button should appear. Transferring Students info should not appear.
Transferring Students info should only appear, if under Immigration Info, they select “I plan to transfer…”.
Your instruction “On regards of displaying only the submit button if the answer to the "Please indicate your plans for the September/October session below.", is equal to "I plan to return", you nee to do the following changes:” but I there are no changes given to go along with this instruction from you.
I am unable to correct these things because the conditions listed are too confusing, but I will again mention that the form worked correctly when we sent it in May. As I said in my email this morning, we only updated the session months and date, which should not have an impact on the conditions that we initially created.
I would like Jotforms to make the corrections needed to allow this form to work correctly. Thank you.

    Betty,

You may also want to try first cloning the demo form https://form.jotform.com/72005908180956 of BDAVID so that you can review it on your Form Builder.

You will be able to see what were changed in the Conditions.

Follow the user guide on  : How-to-Clone-an-Existing-Form-from-a-URL

Let us know if this did not work for you.
