Why is my form disabled?

I just tried to revise my forms to include a Session ID so applicants can re-access their submissions.  Now it says my forms are disabled but I am not over any of my limits.  Can you please assist?

I was making the following changes before I received this error.  The form we send out for our recipients to submit is titled 'Grant Setup Form'.  In order to keep the same link but now include the 'Session ID', I cloned this form and titled it 'Grant Setup Configuration Form', I changed the first page of the 'Grant Setup Form' to record the Session ID information (Name and Email), hid all other fields in the 'Grant Setup Form' so I won't lose the previously submitted data, and linked the 'Thank You Page' to the new cloned form ('Grant Setup Configuration Form').  As soon as I completed this it told me my form was disabled.

It looks like our phishing detector mistook your Session ID form for a phishing for and suspended your account.  I whitelisted the form and activated your account.  The form should no longer cause any trouble.
