I prepared three forms for my church and added them to their web site. I did screw up on my log in. But, the re-set did not work. Help...

I did check your account "mdcrawford" and did not find any issue with that. It seems one of my colleague have already taken care of your account issue and activated your account. Your account is already active and all your forms are back online.
