Is there a way to embed an audio file (mp3) into a form?

I would like to be able to provide an audio file in the form itself so that my users do not have to leave the form (open a link to another web page) to listen to it. 

I would suggest using soundcloud.com. You can upload your mp3 files here.

And then use this soundcloud widget to embed the mp3 file in your form. http://widgets.jotform.com/widget/soundcloud
