Do you have a registration form for hotel guests with CC input?

Looking for an online form that a hotel guest can fill out, connect through square to input a credit card for a deposit. The CC would not get charged right away, just for incidentals. There would also be some custom text to have them review and sign manually on the screen that they acknowledge the rules of the deposit.
