How to change the title of the form?

Hi

I am creating a job adoption application. I used a form you already had in the data base and I changed it for my company.  For some reason it still says TLC Rescue: Dog Adoption Application at the top of the page when I send out the form. How do I change it to my name?

I have checked your JotForm and I am not able to find the old title provided in the form settings or email. It looks like that you have already figured it out. If you need any further assistance or referring to something else, please let us know. We will be happy to take a look again.
