When a user completes and submits the form it is forwarded to Insightly as a Lead, but I two Lead entries in Insightly for the same form.Page URL:

I tested our Insightly integration and I wasn't able to replicate this issue, only one lead showed when I submitted the form:
You could try configuring duplicate detection in your Insightly account: https://support.insight.ly/hc/en-us/articles/204248460-Duplicate-checks-when-importing
