My account is suspended and our forms are not active on our website any longer. Please let me know what I need to do to reactive my account asap.

Your account was suspended because your form http://www.jotform.us/form/70193850773158 is asking for credit card information.
As per our terms of use, that is not allowed.
I had to delete those guilty fields in order to reactivate your account. Please refrain from creating forms with such fields to avoid being suspended again.
You can use one of our payment fields to safely process payments.
