I do not think that the issue is related to JotForm, considering that your webpage can not be displayed and not a standalone link of your form.
I was also not able to replicate mentioned issue. Your website and the form are loaded correctly on my end in all browsers.
I see that your user is using the Internet Explorer. If the issue still persists for your user, you can suggest him/her to use some other browser or to update the Internet Explorer to the latest version.

bmonkeeReplied on August 2, 2017 at 2:43 PMThanks but it seems in the screen shot you have that the webpage is loaded?
Note the word ‘Menu’ at the top. It is a very basic webpage and the form is
99% of it’s makeup.
Please advise.

The webpage was loaded on the screenshot I provided, but I was using Internet Explorer 8 and it seems that your website is not compatible with Internet Explorer 8.
In Internet Explorer 11 the ‘Menu’ is loaded properly.
Like I previously suggested, please tell your user to use some other browser or to update the Internet Explorer to the latest version.

the same user also tried to visit the page on google chrome. it must be an issue with the jotform itself, as all other pages on the website load (pages without jotform) ?

I have just tested your form and, like my colleague, I'm not able to replicate the issue you are mentioning, the form seems to be loading properly on my end, I've tested using Chrome and Internet Explorer, the latest version of both browsers.
I have noticed that you have embedded your form using the iFrame code, this basically loads the form from a different page. Does the issue happens when you open the direct link to your form?
I would also recommend you to try while connected to a different network, may be that the current network has some filters.
Please let us know how it goes.
