Some of my form questions does not display correctly

    Hi, I'm having trouble with some questions. They appear to be OK on the editor, but the actual form does not show the possible answers (questions 2, 3, 5). You can find the print screens attached.

    Additionally, I have a table type questions with multiple value entries. Is there a way to let the following question appear if it's blank, and remain hidden when at least one space has been completed?

    Your form fields appear to be showing up correctly in the preview. Only one field is showing up in the form submissions page because that's the only field that is being filled when you submit the form. I have tested your form, the fields are there. Please check.
