I informed a rep last month that I did not want to continue with this subscription and now I need to be refunded the $19 I was charged to renew this subscription. I was very clear about the ongoing issues that were not resolved and that we wouldn't be using the form or this form service any further. We have since subscribed to a different form service that fits our needs better.

Subscriptions can be cancelled at any time from the account billing page. Here is how to do so for future reference:

I have cancelled your subscription and refunded your last payment. You should see the refund of $19.00 posted to your card in the next 5-7 business days.
