Hi, is the free account permit you to create 5 forms per month or is that the total amount of forms you can create using the account?

Similarly, does the bronze account allow you to make 25 forms per month or is it an annual limit?

That's the total number of forms you can create using one account. It doesn't reset each month. 

Your submission counter (100 per month) and your views counter (1000 per month) automatically reset to zero on the 1st day of each month.

bit of  a follow up question, once you've used up your five, can you delete one and make another one?
