Receiving an error when signing the form with DocuSign widget?

We have included a DocuSign widget to our form- I just received word that  people attempting to sign get an error message: "Something's Wrong" 

I just tested your form and I was able to sign it:

I think I fixed it.  I was getting the error on Chrome but when I checked the widget it appeared to be authenticated, but when I switched to Safari it showed that it needed to be authenticated so I did that then it seemed to work on both browsers.
