New Layout: Why does embedded form display empty on mobile devices?

on desktop everything looks good, but when I tried to open this URL on my iPhone and it looks empty. 

I have just tested your webpage using a mobile browser emulator and noticed the form displays without any problem:

Is the issue still happening on your end? 

I would recommend you to check the website responsive CSS code in order to make sure there is not any rule hiding the JotForm form. 

If you are using a website builder I would also recommend you to check the page settings and make sure any element is hidden while the page is loaded from a mobile device. 

If the problem persists, please provide us details about the device you're testing on, we will try to test on the same device in order to know what may be causing the issue.
