But if you look at column D, E and F these are unfilled

Ref line 5, as you can see dates 28-07-2018 and 10-08-2018 is not shown on the spreadsheet.
This ref to line 4 on the spreadsheet
As you can see column C shows Yes but no data for column D should read 08-09-1017
Otherwise, everything seems to be ok, it is great addon so simple to look at the spreadsheet.
Can you advise me how I can get the data to show?

The issue is happening because you have modified the column header names of the Sheet used in the google integration.
The column headers should match the field names in the form for the integration field mapping process to work properly.
To fix, remove the current integration and then redo it.
The integration process should generate the spreadsheet column headers matching to those in the form. We recommend not altering or renaming the spreadsheet Sheet column header names to avoid disconnection in the form google integration.
