Why won't my forms send confirmation emails like the used to?

    I have set up a form that used to send emails after submission. Recently it has stopped sending emails after submission. I don't know what changed. 

    For example, if I select "Fidelity & Guarantee" as my "Carrier Selection", it should send a confirmation email to the conditional email addresses and the email address entered in the "Email" field. It only sends confirmation to the email address in the "Email" field instead of that one and the conditional emails. Is there a conditional logic error somewhere I'm missing? The spelling is all the same. It worked in the past but no longer does. The emails are not on my "Bounce" list.
