I just checked your site and it's build with Jimdo, an online website builder right? Just in case I'm correct then here's a guide on how to add form with Jimdo page: http://www.jotform.com/help/136-Add-a-form-in-a-Jimdo-page

Hope this works for you. Have a great day!
