I just see an option to cance, upgrade or downgrade but don't see a way to change the type of payment I use. I currently use PayPal but my account is frozen. Can I chage to another form of payment?

Since you were paying using Paypal you need to cancel your payment and then re-upgrade using a different payment method. Here is a guide on how to cancel your subscription.
1. Log in to your PayPal account
2. Find the latest payment you have made to JotForm
3. Click on the subscription number for that transaction
4. Click on the "Cancel Subscription" button.
