How to add logo to my form?

What type of files are accepted to add a Front Page LOGO 

All common image file types are accepted. JPG, PNG, JPEG. You are using our new form layout. In the form builder, click the Add Welcome Page bar at the top then you should be presented with another configuration section to which the add logo option is included.
