Why am I exposed to auto suspend how this please open my account

I checked your forms and it looks like one of your forms and it looks like quite a few of them reference either creating or providing passwords as well as requesting different security codes.  We do not allow for the collection of such data through forms.

We would need to remove the forms that request such information before restoring your account.  If that is acceptable, let us know and we can do so.

To protect users filling out forms, we simply do not allow for asking for usernames or passwords.  We do not make any exceptions to this at this time.  If you require collecting such information, you would need to do so via other means.
