My account has been suspended. Kindly make the forms active. I have actually made forms before. But I have been told that there won't be any problems with the forms but in order to create new forms I might need to register for another plan. But as I can see I haven't made any new forms but at a time I was out of storage space but now it's been solved. So kindly look into this matter and help me remove the suspension so that I can have a good service.Page URL:

We are not able to confirm your association to the company you presented in the form. So this appears to be a scamming scheme. We cannot reinstate this account anymore due to severity of the violation.
I see you created and registered another account again. We will allow you to register another account but be mindful that another violation to our Terms can cause the account getting suspended again and possible permanent ban in using our service.

i haven't created such form in JotForm this might be done by someone else maybe someone might have hacked my account. You can delete this form.I am not sure how this form got into my account. i am completely aware of the terms and conditions kindly help me get my forms back please.

After further investigation, I found that the form was created since 3 years ago and was not ever been used. Your explanation is believable. You must have just unknowingly cloned the form into your account.
I have now re-activated your account/username MathewK and forms. You can log in to the account to review the updated status.
