    after i copy the form html, where do i paste it on the website?

    You need to open your webpage editor, the form should be embeded within the body tags, like this

    <body>EMBED CODE HERE</body>

    and depending of the style, you might probably want to add within the body tags the div tags 

    <div>EMBED CODE OF THE FORM HERE </div>

    So it would look like

    <body><div>EMBED CODE OF THE FORM HERE </div> </body>
