It is against our Terms of Use to collect credit card information in such fashion and for that, you could only use one of our payment integrations: https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm
The only way for us to restore your account is to remove the mentioned fields.

I removed the credit card fields from your form and activated your account. Your account and forms should be usable once again.
