How can I create a contact form that suits my purposes

    Dear people at jotform,

    Hope all is well with you. Have just made a test form and building a jodform does indeed seem to be very easy. However, I have some questions prior to my chosing jotform. It is like this.

    A) I have 3 email addresses on my website:, and and would like to use 1 contact form. Meaning, whichever of the email addresses is clicked the message is send to the respective address from one and the same contact form. Any problem?

    B) I need an additional field (hidden) for the purpose of tracking affiliates. Any problem?

    C) Since I am living and working in Myanmar transfering USD or any other 'hard currency' out of the country is almost impossible. For this reason I am keeping a USD deposit with a relative in the States who pays for me from that money whenever necessary. Any problem

    I would love to receive an early answer concerning the a.m. questions.

    Hi Uwe,

    Welcome to JotForm. Here's what I can suggest in order to achieve that.

    A. You can use 1 contact form with a hidden field called "recipient" and then prepopulate this recipient field with this 3 email addresses. And then use autoresponder to be able to received responses from your form users. Check this step by step guide:

    1. Add hidden box to your form and name it "recipient" (without quote).

    2. Get your form embed code, I'd suggest to use pop-up embed code as this is best for selecting emails to be contacted, I guess :). Check this screen to learn how to get your form pop-up embed code (you can also use other embed options).

    Example code:

    Before implementing this code to your site, you have to add value to your hidden recipient field by prepopulating the 3 emails using URL parameters on each pop-embed codes.

    Now when your form users click one of these emails and submit the form, the selected email address will be able to get user submissions. This would be all possible with the help of Autoresponder. Just setup your autoreponder and selectrecipient as your Recipient E-mail as shown below:


    B. Yes that's possible. You can check the first screenshot to learn how to add hidden fields to your form. If you have further questions about this, just let us know.

    C. That's not a problem. You can let somebody to pay for your subscription if you're going to subscribe with us.

    problem with following the procedure you are suggesting. However, I do
    first need to know whether the contact form as specified in my previous
    email can be integrated into my website (,
    page 5) in size and colour without my having to change anything in terms of
    design and colour theme. Also, as offered by Elton and mentioned in my
    email to him, I would like to know what exactly,

    What I mean is "I can make the form changes for you, if you're having trouble with the guide and if you allow me to do it." But, I guess it would be nice if you'll try it yourself so you would atleast experience and explore more with JotForm. :)
