Contact form no longer works

Hi there! I designed a website for a client years ago when I was still doing web design and they have just contacted me letting me know the jotform-built contact page no longer works. I haven't done any HTML coding or any web design in 5 years so I'm feeling pretty rusty and am not sure how to fix this. Please help!

I see that the form embedded on your web site is associated with the account TwoPeas. This account is registered using the email address info@la*****en.com. If you do not remember the password, you may request for a password reset from the login screen so that the password reset link will be sent to the account registered email address.
