I added the widget "spreadsheet to form" and the item that I am using for the "unique code" is a member ID number.  Once the form is submitted, all the data is downloaded with the exception of the member ID so i'm unable to track it back to the user.

Can you please assist with this issue. The ID number is needed.

What is the form where you have added the Spreadsheet to form widget? You may add a second field for the ID, and duplicate the column ID of your spreadsheet to populate the IDs in the new field.

"Its two forms, 49th NAACP Image Awards Nominating Committee Industry and NAACP.  Would they have to enter the ID again, or can it be automatically duplicated?"

No, the idea was to duplicate the ID column of your Excel file: https://support.office.com/en-us/article/Move-or-copy-rows-and-columns-3ebbcafd-8566-42d8-8023-a2ec62746cfc

And to create a second field where the duplicated "column ID" will be populated.

Finally, re-upload the Excel file into your Spreadsheet To Form widget: https://www.jotform.com/help/442-How-to-Use-the-Spreadsheet-to-Form-Widget

However, it should not be necessary to do that, as the ID should be sent in the submission, and should show up:

    But, it is submitted empty:

I have reported this but to our back-end team, you will be notified via this thread when it gets fixed.

Our developers have implemented a new option called "Submit Input Field value" that you need to enable through the widget's settings in order to send the ID in the submission:

Please enable it, and let us know if you need anything else, we will be glad to assist you.
