Can I link conditional responses to a specific follow up email?

I am tring to send a different email to applicants based on whether or not they are eligible for our program.  I am able to use the conditional formatting to send them to an alternate Thank You page, but can I use conditional formatting to send them a different follow up email?

Yes, that's possible but you will have to create different email notifications then trigger them based on your email condition.
