Condition to show form fields are not working.

I am using the condition show/ hide, but when publishing the form . the fields I chose to show, are not showing.

I did check your form and found that you have several action items to show form field. The issue is that you have added same fields in both the condition and that is what is causing this issue. Can you please let us know which are the form fields which is common in both the condition?

There is a better way to organise your condition by adding "Section Collapse" field. You do not have to add so many action items in condition but just adding the section collapse field will show/hide all the form fields in one single condition.
