Why do the submission pdf's look different today than they did yesterday?

When I view the submissions from my form, the information used to be out to the right of the question, but now it is underneath. It is causing some of my submissions to now be a 2 page PDF instead of a 1 page PDF. How can I make it go back to having the information submitted out to the side? What happened? Was there an upgrade or something I can undo?

In the attached file, the first page is the way it used to be and the second page is the way it is now. I need it to go back to the old way. Help! Ugh, it will only let me upload one picture. The one I'm uploading is the wrong way. The submitted info needs to go to the right of each header.

I cloned your form and added back the notification email. And attached PDF was fine.
