    Hello,

We are an education establishment and run regular events for teachers.  I have create the following form:

    https://form.jotformeu.com/72472619485365

Teachers complete this form to secure a place on a course.  However, some course are limit to 150 delegates.  In some schools there may be 5 teachers attending from the same school.  Teachers currently need to complete the form 5 times (one submission for each staff member).  These places are then deducted from the remaining places.

If there a way that a teacher can complete a single form listing all delegates attending and then when submitted these come through as individual registration?

    Any help much appreciated.

    Thanks

    Simon

Yes it is possible to achieve your requirement. Here are the steps you need to follow:

#1. Add multiple form fields asking for details of 5 teachers.

#2. Add a dropdown field asking for how many teachers needs to be added.

#3. Add "Conditions" in your form to show / hide the teacher's fields based on the selection of dropdown field.

#4. Add "Quantity Gift Registry" widget in your form and set the limit to 150 delegates.  This widget will only allow max of 150 delegates submissions. Every time form is submitted, the number of available delegates will be reduced.

Please take a look at the following cloned form and see if this works as expected:  https://form.jotformpro.com/72502510370948

Feel free to clone this form for a closer look. The following guide should help you in form cloning:  https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
