Did you receive this alert via email? May I ask which email address this message was sent to?
I have a feeling you have unknowingly created a form under a guest account, and now this form exceeded 20 submissions (the limit for a guest account). That may be the reason for the 'please upgrade' email you may have received.
If that is indeed the case, we will need to move the said form to your account, so it can continue receiving responses.
