Unable to open my form

My form is posted on our school web site.  Everything worked fine last week.

Over the weekend students were trying to access - they got on the link - as soon as they entered their information the whole document disappears.  I tried it today and the same thing is happening - I don't know how to fix it

I did a test submission to your form (http://www.jotform.ca/form/71695362317259) and I was not able to replicate the issue. I was able to open your form and submit without any issues. 

Is the issue resolved? If not, let us know. Please try opening your form using a different internet connection.
