My form is having issue moving to next pages

A form at one of my pages is not completing its purpose, it top half way without sending the information and this is causing a serious issue because that form is needed and its important, i have made over 2 forms for that same issue but but still the same result, the first form didn't validate email and phone number, please help me resolve it as soon as possible. thanks

The file upload field needs to be updated. You have set 64kb as the limit. You may consider increasing the limit to 1MB or more. I was able to submit it, but I noticed that the buttons cannot be clicked because of the form height.

Please try re-embedding the form using the following iframe codes, without the script:
