My account has been auto suspended with no reason as to why and no helpful links or emails to explain why and how to unsuspend.

I have deleted the word "Password" to your form and re-activated your account back to good status. You can now manage your forms and account properly.
You can try using the word Authorization, Key, Code, etc.
