Logo and form are not displaying correctly

    Alignment is off and the logo is not showing

    I checked your form but I was not able to see the issue you are reporting. The logo was properly displayed at the top center of the form.

    Can you provide us more details regarding the issue you are encountering so we can further check?

    Evidently, it is an issue with my browser or an extension I have on it, sorry for the confusion.
