Is this related to a form you have created on your account or a payment you did to upgrade your JotForm account?
If this is related to a payment you did through a JotForm form, please note that we are only a form provider service, we are not able to manipulate the information our users receive through their forms. You may need to contact the form's owner in order to ask any questions you have.
