I have added multiple check box fields but they the fields that are checked do not show up on the report

Inside of which report they are not shown?
I checked the submission page of your form and selected checkbox options are shown as dots '.'.
Inside of an Excel report that I created, they are also shown as dots.
Your checkbox options inside of the Configurable list are empty. I would suggest you to name them, so that you can easier see inside of the reports which options were selected.
