Need help with a form emailing issue

There are a couple things we are struggling with when building our form you can see here:

1. When it emails, the form itself is very small

2. Is there a way to set up a copy of the invoice to email back to the sender? 

I checked your page and the embedded form looks quite large:

If you are emailing a link to your page, the form should look the same.

To send a copy of the form data to the user who filled out the form, add an email field to the form and then set up an autoresponder email:

When an email field is added to the form an autoresponder email is added by default.
