I have been checking this and I can see the same you're reporting, first off, your form is cloned differently since the "Form Layout" option is checked on your account, I would recommend you to turn off the option on your cloned form so you get the old layout.

You will be able to change to the old layout on the settings page of your form, select the legacy layout option on the form layout drop down:

Now, regarding the issue with the templates page, I will forward it to our second level so our developers can resolve it, we will let you know as soon as we receive any updates about it.

    Hello yoyitogero,

If you would like to use the templates from the ones that you see while you are logged out (legacy layout templates), you can do so by unchecking the checkbox for Form Layout on your account's settings page: https://www.jotform.com/myaccount/settings

    Thanks.
