Locked out of account

    Ive been locked out of my account and my forms disabled that I had embedded into my website. How do i get this restored?

    You can use Forgot Password button at www.jotform.com and on the bottom right of the page:

    Your account tinasmithportraits is suspended because of the forms WEDDING INFORMATION AND CONTRACT and Clone of WEDDING INFORMATION AND CONTRACT. Inside of this forms you have added the fields that are used to collect credit card info and that is against our Terms of Use.

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    In order to activate your account, it is required to agree to our terms. Also, mentioned fields have to be deleted.

    Please let us know if you agree with our terms and we will delete the guilty fields and activate your account.
