My account has been suspended how can i get it unsuspeneded?

I can see your account has been suspended since you're asking your users to create a password on your form, please note that asking for such information goes against our terms.
In order to reactivate your account we will need to remove the guilty field from your form, please let us know if that's OK for you so we can proceed.

I went ahead, deleted the field and reactivated your account. Please also avoid to ask for information that goes against our terms, this way you avoid your account to get suspended again.
