You need to edit your payment tool of your form and select "Textbox" in "Show Quantity on the Form as" instead of "Dropdown" which will allow user to input custom quantity in textbox. Please check the screenshot below:
Hope this helps.

I will explain how the order process needs to work,
The customer has currently 45 different images they can select from to have printed on a Shower Curtain. In the instructions I recommend they save the desired image or images to their Favorites. They need to record the filename on a piece of paper. On Checkout (The Order Form)I need to know the Qty and Description (filename) for the item:EX: 3 item one's filename 1 item two's filename 5 item three' filenamethen display the totalThen go to PayPal to finalize the order. The transaction sequence works now except I need to know the filename of the image they are interested in.
I could have a separate text box where they could enter the image names, but the form would be a lot cleaner if the qty and item description were on the same line.
I believe that you are referring to the form http://www.jotform.us/form/72668359184167 in your account. I have cloned this form to my account and see that the fields are added to the form to select the quantity and price. In order to calculate the total of the price, you may add the form calculation widget and add the fields to perform a calculation. Please refer to the guide below that can help you with performing calculations.

You may pass the calculation value to the payment field by following the steps in the guide below:

Please give it a try and let us know if you need any further assistance. We will be happy to help.
