I need one form with multiple functions

    I have approx 10 service products that I wish to sell online.

    I will be using a shopping cart online to take payments, however, once a purchase has been made I can only redirect my customers to 1 URL. 

    So I need a form where I can have multiple products on a dropdown list, BUT the customer MUST have the ability to give us details / upload logos or pictures / or even an mp3 file if necessary.

    Do you have a template that can be used for this?


    For example below are just some of the services we will be offering:

    1. Design of Business logo (so would possibly need customer to give us colour details for instance)

    2. Design of business Card (so would possibly  need logo uploaded & company details)

    3. Promotional video (so would possibly need up to 10 jpeg images to be uploaded)

    4. Custom social networks - Facebook, Twitter, Linkedin & Youtube (so would possibly need customisation details)

    5. Complete business start up package (so would require a lot of details in advance fromthe customer)


    What I need is one form, that I can integrate sub categories in to (on a drop down list), that customers can add their unique details to before submitting. 

    I see that you aleady have a form where you can for instance use for ordering a branded facebook page. I would want to potentially use that one, BUT it would have to be one of the forms in a drop down list.


    I hope that makes sense.

    The website is not up and running yet, (because I cannot launch the site untilthe forms are embeddd) so I cannot provide you wth a URL,

    Hi and thanks for contacting us.

    What you need is possible to do with JotForm. I may suggest you to search for first a form template that you prefer (from our Form Templates section).

    Once you find the template you prefer we can work together on the creation of the form

    if you can not find any template that you prefer is always possible to create a form from scratch and customize it as you need.
    I think it's completely useless to start to give instructions or advice even now as confuse your ideas.
