Everything shows up on the form emails witht the exception of the comments section. Not sure why?Page URL:

Hi, this form was created with another account different from yours. By the way after checking I can see that in this email notification missing comment field.
I may suggest you (if you are this form's owner) to remove this actual email notification and create it again. This will solve your problem. Thanks. Alessandro

Hi, and in addition to the comment above...
You can add the a field to your submission notification by editing the content of the notifiction.
If you open your notification, you will see that you have only included the "Full Name" "E-mail" & "Phone number" fields. You can add the "Request/Question" field by selecting it from the menu on the right.
