My busisness provides a service for dentists that is a yearly subscription. For our purposes they can "buy" zipcodes on a map to be listed in. In addition to that they can buy either 1 or 2 slots in each zipcode. Also the first zip they buy costs $299, the second $279 and the third and all after that $249.
We need a way on our form for the customer to be able to enter the first zipcode (thier primary) in a text field with a drop down of 1 or 2 for slots next to it. Then the same thing for the second, third and so on. But what we also want is a way to have all the zipcodes varrying costs to be calculated into a total automatically as the customer enters the info. So if they take 1, 3, or 7 zip codes it will know that the first costs 299, second 279 and the rest 249 and not just charge them for 7 at 299.
So to sum up, on my end i need to know what zip codes their taking and how many slots in each. And on their end they need to be able to enter in that same info and have the correct total calcualted for them so the dont have to do it themselves.

Does this form exemplify what you're trying to achieve?
The said form uses conditions and a payment tool/module (Paypal) to calculate the price.
