How to create a Login form?

Unfortunately, you cannot create login forms on JotForm. This is considered phishing and results in the suspension of the account by our anti-phishing filter tool.

In the demo form, I use the Show or Hide condition. The fields will only show if the "Access Number" field data is correct.
