AUTOSUSPENDED - locked out of account

    My apologies for the duplicate post. I've been suspended and would love to regain access. Please advise. The email on our account is marketing@civcort.com but I would also like to be contacted by my email in the form.

    Your account CIVCORT was autosuspended by our phishing system because of the password fields that you added to your forms.

    It is against our Terms of Use to collect login credentials, such as a password. 

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    I deleted the password fields from your forms and I activated your account. Please do not create the similar forms or your account will be suspended again and we will not be able to activate it.
