I cloned your form to test it further and I was not able to replicate the issue. I did a test submission and it was successful. Here's a screenshot:
I will clear your form cache from our server. Please check it again on your end if the issue persists.
I also tried resizing the Header image and I was not able to replicate the issue you're referring to. May I know what internet browser you're using and what device?
I am using Google Chrome and Windows 10.
